Understand which appraisal form to use and organise your preparation with a simple working template.

Which form should I use?

Ask your appraisal team or appraiser which form or portfolio platform they accept before completing your paperwork. NHS England provides a Medical Appraisal Guide (MAG) model form and accompanying guidance. Check the official page for the available version and instructions.

A working preparation template

Use the prompts below to organise your notes before transferring them into your agreed appraisal form. This is a preparation aid, rather than a replacement for a required platform, declaration or official form.
